SUBJECT: Rider Orders for the 2010 Women's Health Calendar
         
The Government Printing Office (GPO) is now accepting rider orders for the 2010 edition of the Department of Health and Human Services publication Women’s Health Calendar (English & Spanish).

The rider rate for this publication is estimated at $ 1.00 per copy.  Please remember to indicate the number of copies required and GPO Jacket 358-564 (English) and HHS Requisition Number 0-01626 and GPO Jacket 358-565 (Spanish) HHS Requisition Number 0-01044 on your requisition.  Field or regional activities should coordinate their requirements with their Washington, DC area headquarters’ printing officers before submitting any paperwork.

Rider requisitions (SF-1’s) for this product should be submitted to GPO no later than November 25, 2009.  The rider rates apply to single destination, local delivery.  Mailing charges, if incurred, will be added to your account. 

Requisitions should be submitted by one of the three following methods: addressed and mailed to Agency Publishing Services, Requisition Section, Room C825, Mail Stop CSA, U.S. Government Printing Office, Washington, DC 20401; emailed to requisition@gpo.gov; or efaxed to (202) 478-1762.  Your agency’s authorized personnel as reflected in our agency signature file must sign all requisitions submitted.
