TO: Printing and Publishing Officials of the Federal Government

SUBJECT: Rider Orders for the 2011 Women’s Health Calendar

The Government Printing Office (GPO) is now accepting rider orders for the 2011 edition of the Department of Health and Human Services publication Women’s Health Calendar (English & Spanish).

The rider rate for this publication is estimated at $ 1.00 per copy.  Please remember to indicate the number of copies required and GPO Jacket 366-928 (English or Spanish) Requisition Number 1-01084 on your requisition.  Field or regional activities should coordinate their requirements with their Washington, DC area headquarters’ printing officers before submitting any paperwork.

Rider requisitions (SF-1’s) for this product should be submitted to GPO no later than January 10, 2011.  The rider rates apply to single destination, local delivery. Mailing charges, if incurred, will be added to your account. 

Requisitions should be submitted by one of the three following methods: addressed and mailed to Agency Publishing Services, Communication & Status Center (COMSTAT), Mail Stop CSA, U.S. Government Printing Office, Washington, DC 20401; e-mailed to requisition@gpo.gov; or faxed to (202) 478-1762.  Your agency’s authorized personnel as reflected in our agency signature file must sign all requisitions submitted.
