GPOExpressSM Registration

Step 2: SF-1 - GPO Form 3001

To get started simply download the GPO Form 3001 ( FY13 3001), the registration form specifically created for the GPOExpress program. If you would like to apply using GPO's standard SF-1 this is also approved.

GPOExpress Form 3001

On page 3 of the GPO Form 3001 (FY13 3001), you will see a list of thorough instructions for completing the form. For those customers using a Government Purchase card to register for the program, boxes 4-7 do not need to be completed. If you have any questions on completing the Form 3001, please contact your GPO National Account Manager for further information.

When your Form 3001 (FY13 3001) , is completed and signed, please fax a copy of the form to the GPOExpress registration fax at 202.312.0171. Upon submission you will be contacted by your GPO National Account Manager and walked through the remainder of the registration process including the discussion of when your GPOExpress cards will arrive and the optional training resources available.
