SUPPLEMENTARY INFORMATION:
Title: Report of Medical Examination for Disability Evaluation, VA
Form 21-2545.
OMB Control Number: 2900-0052.
Type of Review: Extension of a currently approved collection.
Abstract: VA Form 21-2545 is completed by claimants prior to
undergoing a VA examination for disability benefits. The examining
physician also completes the form to record the findings of such
examination. An examination is required where the reasonable
probability of a valid claim is indicated in any claims for disability
compensation or pension, including claims for benefits based on the
need of a veteran, surviving spouse, or parent for regular aid and
attendance, and for benefits based on a child's' incapacity of self-
support. VA uses the data to determine the level of disability.
An agency may not conduct or sponsor, and a person is not required
to respond to a collection of information unless it displays a
currently valid OMB control number. The Federal Register Notice with a
60-day comment period soliciting comments on this collection of
information was published on May 11, 2011, at pages 27380-27381.
