GPOExpressSM Registration
Step 2: SF-1 - GPO Form 3001
Please complete this SF-1 for each individual BAC number you wish to use to register for the card. Please see sample for more information on required fields.
Effective October 1, 2006, the preferred form will be the GPO Form 3001 (Participation Request) which is tailored to the GPOExpressSM Program. Cards cannot be issued to departments but only to individuals. You can have multiple individuals per BAC number.
For your protection, the credit card number can be left off either form and a GPO representative will call you for the number. After completing the form, fax to 202.312.0171. If unable to fax, call the GPOExpress Program Manager at 202.512.2159.
