Comments are invited on: (a) Whether the proposed collection of information is necessary for the proper performance of the functions of the agency, including whether the information shall have practical utility; (b) the accuracy of the agency's estimate of the burden of the proposed collection of information; (c) ways to enhance the quality, utility, and clarity of the information to be collected; and (d) ways to minimize the burden of the collection of information on respondents, including through the use of automated collection techniques or other forms of information technology. Consideration will be given to comments and suggestions submitted within 60 days of this publication. Proposed Project(s) Title: Welfare Reform Demonstration: Special Application Form. OMB No.: 0970-0134. Description: The information collected by the ``Welfare Reform Demonstration: Special Application Form'' will be used by the Department of Health and Human services to consider welfare reform demonstration projects. Under 42 U.S.C. 1315(a), the Department is given latitude, subject to the requirements of the Social Security Act, to consider and approve demonstration proposals that are likely to assist in promoting the objectives of titles IV-A and D and XIX of the Act.
