Proposed Information Collection Available for Public Comment; 
Request for Payment of Funeral

AGENCY: Director of Information Systems for Command, Control, 
Communications, and Computers (DISC4), U.S. Army.

ACTION: Notice.

SUMMARY: In compliance with Section 3506(c)(2)(A) of the Paperwork 
Reduction Act of 1995, the Department of the Army announces a proposed 
public information collection and seeks public comment on the 
provisions thereof. Comments are invited on: (a) Whether the proposed 
collection of information is necessary for the proper performance of 
the functions of the agency, including whether the information shall 
have practical utility; (b) the accuracy of the agency's estimate of 
the burden of the proposed information collection; (c) ways to enhance 
the quality, utility, and clarity of the information to be collected; 
and (d) ways to minimize the burden of the information collection on 
respondents, including through the use of automated collection 
techniques or other forms of information technology. Title 10, USC 
Sections 1485 & 1486 provides the next-of-kin of deceased military 
personnel a monetary reimbursement of certain costs incurred by the 
next-of-kin in the interment of the deceased.

    Disposition of Remains--Reimbursable Basis, Request for Payment of 
Funeral and/or Interment Expenses, DD FORMS 2065 AND DD

FORM 1375, OMB CONTROL NUMBER (0704-0030).
    Needs and Uses: DD FORM 2065 records disposition instructions and 
costs for preparation and final disposition of remains. DD FORM 1375 
provides next-of-kin an instrument to apply for reimbursement of 
funeral/interment expenses. This information is used to adjudicate 
claims for reimbursement of these expenses.

    Affected Public: Individuals or households.

    Annual Burden Hours: 208.

    Number of Respondents: 1,140.

    Responses per Respondents: 1.

    Average Burden per Response: 30 minutes.

    Frequency: On occasion.
