Agency Information Collection Activities: Request for Emergency 
Review by the Office of Management and Budget

    The Social Security Administration publishes a list of information 
collection packages that will require clearance by OMB in compliance 
with Pub. L. 104-13 effective October 1, 1995, The Paperwork Reduction 
Act of 1995. The information collection listed below has been submitted 
to OMB for emergency clearance. OMB approval has been requested by July 
25, 1997:
    0960-NEW. The information collected on form SSA-6233 will be used 
by the

Social Security Administration to determine whether the payments 
certified to the representative payee have been used for the 
beneficiary's current maintenance and personal needs and whether the 
representative payee continues to be concerned with the beneficiary's 
welfare. The information is also used to determine if the items and/or 
services purchased with funds from dedicated accounts are permitted 
expenditures and if funds are commingled and should count as resources. 
The respondents are individuals and organizations serving as 
representative payees who are required by law to establish a separate 
dedicated account in a financial institution, on behalf of SSI 
beneficiaries, for certain past-due SSI monthly benefits.
    Number of Respondents: 30,000.
    Frequency of Response: Annually.
    Average Burden Per Response: 20 minutes.
    Estimated Annual Burden: 10,000 hours.
