Proposed Data Collections Submitted for Public Comment and 
Recommendations

    In compliance with the requirement of Section 3506(c)(2)(A) of the 
Paperwork Reduction Act of 1995 for opportunity for public comment on 
proposed data collection projects, the Centers for Disease Control and 
Prevention (CDC) will publish periodic summaries of proposed projects. 
To request more information on the proposed projects or to obtain a 
copy of the data collection plans and instruments, call the CDC Reports 
Clearance Officer on (404) 639-7090.
    Comments are invited on: (a) Whether the proposed collection of 
information is necessary for the proper performance of the functions of 
the agency, including whether the information shall have practical 
utility; (b) the accuracy of the agency's estimate of the burden of the 
proposed collection of information; (c) ways to enhance the quality, 
utility, and clarity of the information to be collected; and (d) ways 
to minimize the burden of the collection of information on respondents, 
including through the use of automated collection techniques for other

Proposed Project

    Translating Research Into Action for Diabetes (TRIAD)--New--The 
National Center for Chronic Disease Prevention and Health Promotion 
(NCCDPHP), Division of Diabetes Translation. Diabetes exerts a huge 
public health burden, and there are several efficacious interventions 
to combat the effects of this disease. Yet, the quality of care and 
quality of life among people with diabetes remain sub-optimal in the 
United States. The Centers for Disease Control and Prevention, Division 
of Diabetes Translation, intends to conduct a multi-center study called 
Translating Research Into Action for Diabetes (TRIAD). This study will 
assess quality of diabetes care and identify effective means of 
applying existing knowledge to improve care and quality of life. Data 
provided from TRIAD will be critical to the Division of Diabetes 
Translation's ongoing efforts to reduce the burden of diabetes.
    Data will be collected through cooperative agreements with six

Managed Care Organizations (MCOs). Each MCO will provide data on 
organization of care, details of diabetes interventions, and efforts to 
improve diabetes care collected from interviews of health plan and 
provider group directors. Each MCO will also provide data from random 
samples of its diabetic members collected via computer-assisted 
telephone interviews (CATIs). Information from plan members will 
include demography, quality of diabetes care, quality of life, 
satisfaction with care, diabetes severity and duration, and barriers to 
care. Each MCO will collect data from directors and diabetic members at 
baseline and after two years of follow-up. The total cost to 
respondents is estimated at $1,620,000.
