ORDER signed by Magistrate Judge Gregory G. Hollows on 7/29/08 ORDERING that plaintiff shall submit, within 30 days from the date of this order, a completed affidavit in support of his request to proceed IFP on the form provided by the Clerk. The Clerk is directed to send plaintiff a new Application to Proceed IFP By a Prisoner. Plaintiff shall submit, within 30 days from the date of this order, a certified copy of his prison trust account statement for the six month period immediately preceding the filing of the complaint.
