WRITTEN Opinion entered by the Honorable Joan B. Gottschall on 9/4/2012 Plaintiff's motion for leave to file in forma pauperis [#3] is granted. The Court orders the trust fund officer at Plaintiff's current place of incarceration to deduct $5.80 from Plaintiff's account for payment to the Clerk of Court as an initial partial filing fee. The Clerk is directed to forward a copy of this order to Supervisor of Inmate Trust Fund Accounts, Cook County Dept. of Corrections Administrative Office, Division V, 2700 S. California, Chicago, IL 60608. However, Cermak Health Services is dismissed as a Defendant. The Clerk is directed to issue summonses for Defendants Dart, Johnson, and Cox, and the United States Marshals Service is appointed to serve them.
