MEMORANDUM AND ORDER re:3 MOTION to Appoint Counsel filed by Plaintiff Eugene K. Jones-El, 2 MOTION for Leave to Proceed in forma pauperis under 42:1983 (prisoner) filed by Plaintiff Eugene K. Jones-El. IT IS HEREBY ORDERED that plaintiff's motion to proceed in forma pauperis [Doc. 2] is GRANTED. IT IS FURTHER ORDERED that plaintiff shall pay an initial partial filing fee of $1.50 within thirty (30) days from the date of this Order. Plaintiff is instructed to make his remittance payable to "Clerk, United States District Court," and to include upon it: (1) his name; (2) his prison registration number; (3) the case number; and (4) that the remittance is for an original proceeding. IT IS FURTHER ORDERED that plaintiff shall submit an amended complaint, in accordance with the instructions set forth in this Memorandum and Order, no later than thirty (30) days from the date of this Memorandum and Order. IT IS FURTHER ORDERED that the Court shall mail to plaintiff a blank form for the filing of a prisoner civil rights complaint. Plaintiff may request additional forms from the Clerk, as needed. IT IS FURTHER ORDERED that plaintiff's motion for appointment of counsel [Doc.3] is DENIED, without prejudice. IT IS FURTHER ORDERED that if plaintiff fails to comply with this Order, the Court will dismiss this action, without prejudice and without further notice. MEMORANDUM AND ORDER: IT IS HEREBY ORDERED that the Court's previous March 8, 2016 orders granting plaintiff leave to proceed in forma pauperis and assessing a $1.50 initial partial filing fee are VACATED; plaintiff will not be allowed to proceed in forma pauperis in this action. IT IS FURTHER ORDERED that this action is DISMISSED, without prejudice, pursuant to 28 U.S.C. 1915(g). IT IS FURTHER ORDERED that all remaining pending motions are DENIED as moot.
