MEMORANDUM AND ORDER -....IT IS HEREBY ORDERED that plaintiffs motion to proceed in forma pauperis is GRANTED. [Doc. 2] IT IS FURTHER ORDERED that plaintiff's motion to proceed in forma pauperis is DENIED as moot. [Doc. 6]IT IS FURTHER ORDERED that plaintiff's motion to amend is DENIED. [Doc. 5] IT IS FURTHER ORDERED that, within twenty-one (21) days of the date of this Memorandum and Order, plaintiff must pay an initial filing fee of $1.00....IT IS FURTHER ORDERED that, within twenty-one (21) days of the date of this Memorandum and Order, plaintiff shall file an amended complaint in accordance with the instructions set forth herein. IT IS FURTHER ORDERED that the Clerk of Court shall mail to plaintiff a blank Prisoner Civil Rights Complaint form. Plaintiff may request additional forms as needed. IT IS FURTHER ORDERED that if plaintiff fails to timely file an amended complaint, the Court will dismiss this action without prejudice and without further notice as to all defendants except for defendant Jason Davis in his individual capacity. IT IS FURTHER ORDERED that if plaintiff fails to timely pay the initial filing fee of $1.00, the Court will dismiss this entire action without prejudice and without further notice for failure to comply with the Courts orders. MEMORANDUM AND ORDER: IT IS HEREBY ORDERED that that the Clerk of Court shall issue process or cause process to issue upon the amended complaint, pursuant to the service agreement the Court maintains with the Missouri Attorney General's Office, as to Jason Davis, Cindy Griffith, and Alan Earls in their individual capacities. IT IS FURTHER ORDERED that, in their individual capacities, defendants Jason Davis, Cindy Griffith, and Alan Earls shall reply to the amended complaint within the time provided by the applicable provisions of the Federal Rules of Civil Procedure. IT IS FURTHER ORDERED that plaintiff's due process, deliberate indifference, failure to protect, supervisory liability, and equal protection claims are DISMISSED without prejudice. A separate order of dismissal will be entered herewith. IT IS HEREBY CERTIFIED that an appeal from this partial dismissal would not be taken in good faith.. Signed by District Judge Charles A.
