MEMORANDUM AND ORDER rIT IS HEREBY ORDERED that the Clerk of Court should provide petitioner a blank copy of a § 2254 application for filing a petition for writ of habeas corpus. IT IS FURTHER ORDERED that petitioner shall file an amended application for writ of habeas corpus within thirty (30) days of the date of this Memorandum and Order containing only claims relating to his 2002 NGRI plea. Petitioners amended application should be filed on the court-form. IT IS FURTHER ORDERED that petitioners claims relating to his 1991 conviction and sentence are DENIED AND DISMISSED AS SUCCESSIVE. An Order of Partial Dismissal relating to these claims shall accompany this Memorandum and Order. IT IS FURTHER ORDERED that petitioners application for copies of his exhibits [Doc. #5] is DENIED. IT IS FURTHER ORDERED petitioners motion for leave to supplement his petition [Doc. #6] is DENIED AS MOOT. IT IS FURTHER ORDERED that petitioners request for appointment of counsel [Doc. #3] is DENIED at this time. IT IS FURTHER ORDERED that no certificate of appealability shall issue.(Attached a blank copy of a § 2254 application to this order herein.) ( Amended/Supplemental Pleadings due by 12/27/2017.)
