PRISONER PAYMENT ORDER - granting 2 Motion for Leave to Proceed in forma pauperis. When funds exist in the plaintiff's prison trust account, the plaintiff's institution shall collect them and remit them to the court subject to the limitations set forth in this order. The clerk of the court shall serve a copy of this order on the appropriate officer at the plaintiff's institution; the plaintiff shall keep the court informed of his current address at all times while this case is pending. ORDER ON INITIAL REVIEW - To obtain service of process on the defendant, the plaintiff must complete and return forms which the Clerk of Court will provide. The Clerk of Court shall send ONE summons and ONE Form 285 to the plaintiff together with a copy of this Order. The Marshal shall serve the summons and complaint without payment of costs or fees.
